Assertive Community Treatment (ACT) Registered Nurse (4201)

Monitor and oversee all medical and clinical aspects of services delivered to adults with severe mental illness, including psychopharmacologic services, crisis assessments, and symptom management. Provide medication monitoring and documentation, conduct individual counseling, and deliver substance abuse treatment and psycho-educational support within your scope of practice. Collaborate with the multidisciplinary ACT team to coordinate care across providers, assist in creating transition plans for clients moving in and out of services, and provide written documentation for treatment and goal plans.
