Assertive Community Treatment (ACT) Registered Nurse (4180)

Monitor and oversee all medical and clinical aspects of services delivered, including medication monitoring, documentation, and psychopharmacologic interventions. Conduct crisis assessments, manage symptoms, and develop intervention plans while collaborating with the ACT team to track clinical status and treatment response. Provide individual counseling, substance abuse treatment, and psycho-educational support, and assist in creating transition plans for clients moving in and out of services. This role supports individuals with complex behavioral health needs through a person-centered, community-based approach within a stable, mission-driven organization.
