Assertive Community Treatment (ACT) Manager

Job Summary
Provide intensive case management for individuals with moderate to severe mental illness and co-occurring disorders under clinical supervision. Deliver individualized service plans, coordinate multi-disciplinary care, and maintain accurate clinical charts and progress notes within 24 hours of each session. Serve as a liaison between the hospital, criminal justice system, and community resources while ensuring appointment cancellation rates remain below 10%. Complete required supervision, annual training, and maintain appropriate licensure. Travel within Greene County to surrounding areas for client services.
Required Qualifications
- Bachelor's degree in human/social service field, or associate degree with equivalent field experience
- 3 years working with SMD population
- First Aid
- CPR
- De-escalation
- Valid Ohio driver's license
- Proof of automobile liability insurance
- Meets OMHAS licensure and certification requirements
- Competencies for serving the SMD population
- Knowledge of Safety rules and procedures
- Knowledge of agency policies and procedures
- Knowledge of Greene County area and resources
- Knowledge of documentation requirements
