Assertive Community Treatment (ACT) Manager

Job Summary

Provide intensive case management for individuals with moderate to severe mental illness and co-occurring disorders under clinical supervision. Deliver individualized service plans, coordinate multi-disciplinary care, and maintain accurate clinical charts and progress notes within 24 hours of each session. Serve as a liaison between the hospital, criminal justice system, and community resources while ensuring appointment cancellation rates remain below 10%. Complete required supervision, annual training, and maintain appropriate licensure. Travel within Greene County to surrounding areas for client services.

Required Qualifications

  • Bachelor's degree in human/social service field, or associate degree with equivalent field experience
  • 3 years working with SMD population
  • First Aid
  • CPR
  • De-escalation
  • Valid Ohio driver's license
  • Proof of automobile liability insurance
  • Meets OMHAS licensure and certification requirements
  • Competencies for serving the SMD population
  • Knowledge of Safety rules and procedures
  • Knowledge of agency policies and procedures
  • Knowledge of Greene County area and resources
  • Knowledge of documentation requirements
  • Driving
  • Computer literacy
  • Interpersonal skills
  • Capacity to perform duties effectively under potential emotional stress and conflict situations
  • Proficiency in utilizing basic computer skills and office equipment for documentation and communication purposes
