Job Summary

Provide individual and family therapy to clients receiving Assertive Community Treatment services, implementing evidence-based interventions for symptom stability and community functioning. Collaborate with clients, families, and natural supports to achieve employment, housing, and educational goals outlined in Person-Centered Plans. Conduct employment assessments, advocate for clients in the community, and assist with crisis and transition planning. Maintain accurate documentation, including treatment notes and goals, while ensuring fidelity to ACT model standards. Offer consultation to the team on employment and social interaction issues, and provide training to families and community partners. Work onsite or at designated assignment locations, supporting individuals with mental health and substance use disorders in developing their inherent strengths.

Required Qualifications

  • Must meet Qualified Professional (QP) status
  • Bachelors Degree in Human Services/ Pyschology
  • Strong organizational and communication skills
  • Commitment to delivering person-centered, trauma-informed care
  • Ability to collaborate effectively with a multidisciplinary team
  • Must be free from all illicit substance use
  • This position is classified as Light Work (no lifting over 20 pounds)
  • Must be authorized to work in the United States
