Job Summary
Aspire Lead IDD Supports Coordinator provides caseload management and supervision for supports coordination services to individuals with intellectual and developmental disabilities, coordinating therapy referrals and authorizations, assisting with discharge documentation and guardianship processes, facilitating person-centered planning meetings, and advocating for consumers while maintaining compliance with HIPAA and agency policies. The role requires a QIDP or relevant social work/clinical licensure, experience with IDD populations, ability to work in a multidisciplinary team, and proficiency with EHR systems, data reporting, and treatment planning; strong organizational, communication, and problem-solving skills, plus a commitment to inclusive, person-centered care. Based in Detroit, MI with a full-time schedule.
Required Qualifications
- Qualified Intellectual Disability Professional (QIDP)
- Bachelor’s degree in Social Work OR Master’s degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy
- State licensure options (LBSW/LLBSW, LMSW/LLMSW, LPC/LLPC, LMFT/LLMFT, LP/LLP, TLLP, etc.) or psychology licensure
- Experience/training with individuals with intellectual and developmental disabilities; fieldwork/internships related to such populations
- Within 1 year of employment: CMHP and QMHP designations
- Valid and unrestricted Driver’s License
- Proficiency with computers/EHR/MS Office
- HIPAA knowledge in healthcare settings
- Ability to collaborate in a multidisciplinary team and communicate effectively
- Strong time management, problem solving, and interpersonal skills
- Commitment to inclusive environment and person-centered planning
- Willingness to complete ongoing in-service training (minimum 24 hours/year)
