Job Summary
Diagnose patients with intellectual disabilities and conduct treatments within the Stepped Care model, collaborating closely with behavioral scientists and paramedics to create comprehensive treatment plans. Provide preventive care to avoid complications and improve quality of life for clients across intramural and extramural settings in Amsterdam. Coordinate care processes, stimulate innovations, and offer training to colleagues to advance professionalization within the multidisciplinary team. This role involves working in various residential facilities and polyclinics for clients with complex care needs. The position offers a one-year contract with an annual bonus and pension contributions.
Required Qualifications
- Ervarde Arts Verstandelijk Gehandicapten (VG)
- Sterke communicatieve vaardigheden
- Moeiteloos schakelen tussen verschillende niveau's van zorg
- Ambitieus zijn
- De juiste opleidingen afgerond
- Geregistreerd Arts Verstandelijk Gehandicapten (AVG)
- Affiniteit met de zorg voor mensen met verstandelijke beperkingen
- Interesse om binnen het Stepped Care model te werken
- Minstens 3 jaar ervaring in een soortgelijke functie
- Een uitstekende beheersing van de Nederlandse taal
- Een rijbewijs
- Eigen vervoer
