Job Summary
Arrive Home Nurse Practitioner focused on post-acute and chronic care management, delivering high-quality, home-based healthcare. Responsibilities include conducting focused assessments for post-acute patients with chronic illnesses, collaborating with physicians and other providers to coordinate care, guiding clinical staff, educating patients and families on care management and warning signs, ensuring compliance with clinical goals and maintaining medical records, and participating in continuous education to advance home-based wound care and chronic care management.
Required Qualifications
- Minimum of 3-5 years of clinical experience as a Certified Nurse Practitioner (CNP) in direct patient care with experience in treating medically complex patients with multimodal care including pharmacologic and non-pharmacologic therapy
- Wound care experience preferred
- Proficiency in performing assessments understanding disease process of chronic illnesses
- Strong interpersonal and communication skills, with the ability to effectively educate and empower patients and caregivers
- Ability to work independently and collaboratively within a multidisciplinary team environment
- Master’s degree in nursing from an accredited Nurse Practitioner program
- Active, unrestricted Certified Nurse Practitioner (CNP) license in the state of practice, with national certification as a Nurse Practitioner in Adult-Gerontology, Family, or Acute Care
