Job Summary
Develop, implement, and monitor individual program plans while supervising training and day habilitation activities on behalf of consumers. Regularly monitor data sheets to track goal progression, integrate total program plans with habilitation, and ensure case management services meet consumer and family satisfaction outcomes. Communicate with the Interdisciplinary Team regarding consumer care changes, coordinate meetings, and advocate for needed services or crisis interventions. Supervise Team Members through employment actions including hiring, training, and performance evaluations while ensuring compliance with federal, state, and local employment laws. All work exceeding 40 hours weekly requires supervisor approval.
Required Qualifications
- Bachelor's degree in social services field from an accredited university/college or equivalent experience
- Minimum of 5 years working with individuals with developmental disabilities
Desired Qualifications
- Two years supervisory experience preferred
