Job Summary

Respond to individuals seeking assistance through the Aging and Disability Resource Center phone line, completing comprehensive intake, screening, referral, and follow-up services for older adults, individuals with disabilities, caregivers, families, and professional providers. Assess individual needs to provide objective, person-centered information regarding public benefits, community resources, long-term services, and private care options while advocating on behalf of clients with community agencies. Maintain accurate documentation within state data systems, verify client screenings within required timeframes, and ensure all program documentation meets federal, state, and agency compliance standards. Provide Medicare counseling appointments and assist clients with enrollment, savings programs, low-income subsidies, prescription assistance, and appeals.

Required Qualifications

  • Bachelor's degree in Social Work, Gerontology, Psychology, Sociology, Human Services, Public Health, or a closely related field
  • equivalent professional experience working with older adults, individuals with disabilities, or community-based services
  • Compassionate, adaptive communication skills
  • Technical proficiency

Desired Qualifications

  • Experience mastering multiple program knowledge while meeting compliance and reporting requirements
