Job Summary
Provides clinical care within the Nurse Practitioner scope of practice. Assesses patient illness by obtaining complete medical history and recording findings, performs physical examinations, orders laboratory tests, and analyzes results. Diagnoses health needs based on clinical data. Delivers direct patient care as part of a clinical team with Nurses, MDs, Medical Assistants, Case Managers, and Patient Access Representatives to achieve the patient’s plan of care. Collaborates with team members to ensure quality patient care and efficient service. May concentrate in medical specialties appropriate to skills and Board certifications. Participates in community health initiatives and health fairs as needed. Refers patients for diagnostic tests and specialist services as appropriate. Works with staff to plan, develop and evaluate patient care plans and supports community health programs and policies. Maintains professional development and attends meetings and conferences as outlined by the Medical Director. Other duties as requested.
Required Qualifications
- Current APRN license
- CPR certification
- Master’s degree from an accredited program in nursing and successful completion of an accredited certified nurse practitioner program
- Experience as a nurse practitioner in a Community Health Center/Primary Care setting, or an Emergency Department/Urgent Care
- Family APRN training preferred with Pediatric experience, however, will consider other subspecialty training and experience
