Job Summary
Provide total care for assigned patients in collaboration with the physician of record and patient care team. Perform patient assessments, create care plans, and provide interventions for acute, chronic, and palliative needs. Diagnose conditions, interpret x-rays and lab results, write prescriptions, and order home health, therapies, and DME. Address quality measures and HCCs to meet organizational goals. Ensure all conditions are documented in the electronic medical record per policy and CMS requirements. Instruct patients on therapeutic regimens and refer them to community resources as needed. Complete all documentation and billing in a timely manner.
Required Qualifications
- Completion of master's degree in nursing
- Current active Florida License as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA)
- Computer literate with good computer skills
- Knowledge of Athena Database software
- Knowledge of Microsoft Spreadsheet software
- Knowledge of Microsoft Word Processing software
- Ability to read, analyze, and interpret the most complex documents
- Ability to respond effectively to the most sensitive inquiries or complaints
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percent and to draw and interpret bar graphs
- Ability to define problems, collect data, establish facts, and draw valid conclusions
- Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables
Desired Qualifications
- Two or more years related experience and/or training
- Equivalent combination of education and experience
- CPR/BLS certifications
