Job Summary
Provide advanced clinical care to patients with serious and life-limiting illnesses by performing comprehensive assessments, diagnosing conditions, and developing individualized plans of care. Assess and manage pain and symptoms, monitor patient progress, and adjust care plans as needed while collaborating with physicians and interdisciplinary teams. Communicate effectively with patients, families, and providers regarding goals of care and prognosis, offering education, guidance, and emotional support throughout the journey. Accurately document all assessments and interventions in the electronic medical record. Travel throughout the Newark, Columbus, and surrounding communities to provide patient care as needed.
Required Qualifications
- Master of Science in Nursing (MSN)
- Minimum of 2 years of clinical experience as an APRN
- Active, unrestricted Registered Nurse (RN) license in the State of Ohio with no current board actions
- Board certification as an APRN through a nationally recognized certifying agency
- Active, unrestricted DEA license
- Eligibility to maintain adequate malpractice coverage
- Current CPR certification
- Strong computer skills and ability to accurately document patient care
- Valid driver's license with a safe driving record
- State-required minimum automobile insurance coverage
- Ability to travel and drive during daytime, nighttime, and inclement weather conditions
- ACHPN certification (required within two years of employment)
- Certification after completing 500 hours of hospice and/or palliative care experience
Desired Qualifications
- Certification in Hospice & Palliative Medicine (ACHPN)
- Previous hospice, palliative care, geriatrics, oncology, or other serious-illness care experience
- Experience providing care across community-based settings
