Job Summary

Assess complex patient needs, diagnose conditions, and develop individualized plans of care for patients with serious, life-limiting illnesses. Manage pain and symptoms to promote comfort while collaborating with physicians and interdisciplinary teams to coordinate patient-centered treatment. Provide education, guidance, and emotional support to patients and families, accurately documenting all assessments and interventions in the electronic medical record. Travel throughout the Newark, Columbus, and surrounding communities to deliver advanced clinical care. Requires an MSN, active Ohio APRN license, board certification, and ability to drive during all weather conditions.

Required Qualifications

  • Master of Science in Nursing (MSN)
  • Minimum of 2 years of clinical experience as an APRN
  • Active, unrestricted Registered Nurse (RN) license in the State of Ohio with no current board actions
  • Board certification as an APRN through a nationally recognized certifying agency
  • Active, unrestricted DEA license
  • Eligibility to maintain adequate malpractice coverage
  • Current CPR certification
  • Strong computer skills and ability to accurately document patient care
  • Valid driver's license with a safe driving record
  • State-required minimum automobile insurance coverage
  • Ability to travel and drive during daytime, nighttime, and inclement weather conditions
  • ACHPN certification (required within two years of employment)

Desired Qualifications

  • Certification in Hospice & Palliative Medicine (ACHPN)
  • Previous hospice, palliative care, geriatrics, oncology, or other serious-illness care experience
  • Experience providing care across community-based settings
