Job Summary
Deliver in-home and virtual care to patients with complex medical needs as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant). Collaborate with a multidisciplinary team to provide comprehensive assessments, chronic and acute condition management, comprehensive medication management, transitions of care, and address social determinants of health. Participate in team huddles and clinical partnership meetings; ensure accurate documentation and coding; contribute to after-hours on-call coverage and cross-coverage as needed. Requires active PA licensure, DEA registration, and ability to travel to patient homes; in-home and telehealth care delivery with a focus on value-based, patient-centered care for aging adults and underserved populations.
Required Qualifications
- Licensed Nurse Practitioner (ANCC or AANP certification) or Board-Certified Physician Assistant with active licensure in Pennsylvania
- Active DEA licensure and state-specific controlled substance registration, as applicable
- Experience providing care to aging adults or individuals with complex medical needs
- In-home care and telehealth experience
- Ability to travel to locations with limited public transportation; liability and property damage insurance on vehicle used
- Vaccination requirement for frontline workers (as stated)
