APN - High Risk Clinic

Licensed Advanced Practice Nurse to join Starling Physicians’ High Risk Department. The APN will manage complex, high-risk patients with multiple chronic diseases who are in transitions of care or have higher acuity, and will collaborate with Starling physicians and care-management staff to provide medical care and assessment of care management, social work, and pharmacy needs to achieve high-value outcomes related to quality metrics, patient experience, and appropriate utilization and costs. Minimum CT State APN license required; 1-2 years experience preferred.
