Job Summary
Conduct comprehensive assessments, evaluations, care planning, and coordination for assigned adult AOT/Health Home Plus Care caseloads. Deliver outreach, crisis intervention, and advocacy while linking consumers to medical, financial, social, rehabilitation, and psychiatric services. Develop social supports and procure necessary resources. Manage recordkeeping, documentation, statistical tracking, and quality assurance activities.
