AOD Residential Case Manager

Job Summary
Build therapeutic relationships and complete holistic assessments to identify participant needs, risks, and treatment goals. Develop and deliver care plans including treatment matching, outcome monitoring, and aftercare planning while providing evidence-based interventions such as Motivational Interviewing and relapse prevention. Manage risk and safety through assessment, safety planning, and incident response, coordinating care with internal and external services for continuity. Facilitate group programs by delivering one to two groups per fortnight and providing backfill when required. Support transition and aftercare through planned and unplanned exit plans, referrals, and assertive community linkages. This role is part of an on-call roster to provide response for risk matters outside normal business hours.
Required Qualifications
- AOD Skill Set (CHCSS00093), including CHCAOD001, CHCAOD004, CHCAOD006, CHCAOD009
- Relevant qualifications such as Diploma or Degree in Social Work, Psychology, Counselling or related fields
- Experience working with people experiencing complex needs, including AOD use, mental health challenges, trauma, homelessness, and dual-diagnosis presentations
- Demonstrated ability to apply trauma-informed, strengths-based, recovery-oriented, and harm reduction approaches to support positive client outcomes
- Strong case management, assessment, advocacy, risk management, and problem-solving skills, with the ability to work collaboratively across housing, health, and community service sectors
- A Victorian Employee Working with Children Check
- A current Victorian Driver's Licence
- A mandatory Nationally Coordinated Criminal History Check
