Job Summary
AOD Case Manager collaborates on a multidisciplinary team to provide recovery-focused community rehabilitation services for individuals with AOD issues and/or severe mental illness. Responsibilities include coordinating evaluations and ISP development, assisting clients to reduce symptoms, connecting to community resources, supporting social networks, accompanying clients to appointments, ensuring occupancy compliance, and maintaining documentation. Requires a Bachelor's degree in a human services field; chemical dependency license preferred; valid Ohio driver’s license; strong coordination, advocacy, and communication skills. Full-time role based in Akron, OH at Community Support Services with benefits including insurance, PTO, 401(k), and professional development.
Required Qualifications
- Bachelor’s Degree in a human service field
- Chemical Dependency license preferred
- Prior experience in case management with individuals with substance abuse
- Experience with severe and persistent mental illness preferred
- A car and a valid Ohio driver’s license
- Proficiency in Microsoft Outlook
- Ability to work in a team environment
- Knowledge of Housing First philosophy
- Strong organizational and communication skills
