Job Summary
Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive assessments. Document all findings, upload records to the Synergy Software portal, and obtain physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, coordinating significant changes with physicians and the broader home care team. Manage re-certifications, transfers, and discharges while supervising Home Health Aides per regulatory requirements. Assess needs for additional services from physical, occupational, and speech therapists. Work flexible schedules including weekends and holidays, earning $50–$140 per visit as a W-2 per diem.
Required Qualifications
- 1 year RN experience
- 1 year nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
- Ability to work at least two-three days each week
- Ability to work one weekend a month
- Ability to work two holidays per year (one winter and one summer)
- Knowledge of regulatory requirements
