Job Summary
Plan, direct and coordinate the activities of the Social Service program; develop and implement social services programs to meet resident and community needs; conduct needs assessments; provide direct social work services, counseling, and discharge planning; coordinate care plans, document changes, maintain records, and administer program budgets; collaborate with agencies, ensure compliance with health care standards, and supervise staff and volunteers to deliver quality social services.
Required Qualifications
- High School diploma or equivalent; previous long-term care experience preferred; or an equivalent combination of education and experience
- Bachelor of Social Work Degree or approved social service designee course preferred
