Ambulatory Social Worker - Palliative Medicine and Support Care - FT - Day

Conduct psychosocial support for patients with chronic disease through interdisciplinary collaboration, assisting with admission paperwork and serving as a patient advocate. Perform social work assessments addressing bio-psychosocial, cultural, and spiritual ramifications of illness, then develop case management and treatment plans based on multidisciplinary team recommendations. Manage psychosocial problems including conflict, isolation, and advance directives while initiating discharge planning and long-term care coordination. Provide crisis therapy, grief support, and patient education regarding diagnosis and treatment. Liaise between the Palliative care team and community hospice, home care agencies, and external social services to facilitate community reintegration. Participate in performance improvement initiatives and patient care conferences. Maintain confidentiality and comply with professional ethics.
