Ambulatory Social Worker - Care Transformation - FT - Day

Participate in patient admission processes, assisting with voluntary papers and serving as a patient advocate throughout the Stormont Vail Health system. Perform social work assessments addressing bio-psychosocial, cultural, developmental, and spiritual ramifications of illness, then develop case management and treatment plans based on multidisciplinary team recommendations. Initiate discharge planning upon admission, coordinate long-term planning to reduce recidivism, and provide crisis or brief therapy for individuals, families, and groups. Establish therapeutic working relationships, communicate patient status clearly, and provide education regarding diagnosis and treatment while maintaining confidentiality and adhering to professional ethics. Actively participate in patient care conferences and develop new services responsive to patient care demands.
