Ambulatory Social Work Specialist - Primary Care

Job Summary
Serve as centralized triage for ambulatory clinic social service cases and medical appointment transportation needs. Delegate and distribute cases to appropriate UW Health staff, complete documentation and reports, and manage less complex cases including coordination of care and facilitation of community-based referrals. Apply Trauma Informed principles with all interactions. Complete patient needs assessments and analyze chart documentation to optimize interventions addressing social determinants of health barriers impacting adherence to care and patient wellbeing. Full-time, 100% FTE day shift Monday through Friday from 8:30am to 5:00pm.
Required Qualifications
- Bachelor's Degree in psychology, social work, counseling, or other behavioral science related degree
- 1 year related experience
- Experience working with a diverse population base, with complex social/emotional needs
Desired Qualifications
- Previous medical setting experience
