Job Summary
Conduct Medicare Annual Wellness visits in accordance with CMS regulations while providing quality triage and patient care using strong assessment skills and point of care testing. Collaborate with clinicians to assess complex patients, establish care plans, and set priorities with patients and families. Triage and coordinate patient care for both acute and chronic disease management while advocating for patient rights, values, and belief systems. Work closely with the interdisciplinary care team to improve quality and patient satisfaction within Brattleboro, Vermont.
Required Qualifications
- Graduate of an RN accredited nursing program
- Active State of Vermont RN license
- Current Healthcare Provider CPR certification
- 3-5 years recent outpatient clinical nursing experience
- Acute care experience
- Strong assessment skills
- Point of care testing
- Ability to triage and coordinate patient care
- Proactive approach to both acute and chronic disease management
- Ability to effectively lead and coach others
- Strong attention to detail
- Close Vision (clear vision at 20 inches or less)
- Color Vision (ability to identify and distinguish colors)
