Job Summary
Delivers pharmaceutical care through patient-centered clinical services, medication information, education, preparation, and distribution to ensure safe, effective, and cost-efficient therapy. Exercises sound judgment to provide input on and implement patient care plans based on diagnosis while gaining cooperation through competent assessment and effective communication. Adheres to all local, state, and federal regulations to ensure privacy and safety. Provides comprehensive medication management including initial assessments, toxicity evaluations, symptom management, and reassessments. Coordinates and directs work of others to meet specified deadlines while prioritizing competing clinical and operational tasks. Requires flexibility to orient and work at provider clinics and specialty pharmacies.
Required Qualifications
- Graduate of an accredited pharmacy school
- Registered Pharmacist within 120 days
- Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations and laws and regulations
- Knowledge and understanding of different Pharmacy practice settings, including narcotic delivery and procedures
- Knowledge and ability to identify and employ pharmaceutical and medical terms, abbreviations and symbols commonly used in prescribing, dispensing, and record keeping of medications
- Basic working knowledge of legal requirements and accreditation standards including The Joint Commission, Title XXII, Department of Homeland Security (DHS), Drug Enforcement Administration (DEA), Food and Drug Administration (FDA), and United States Pharmacopeia (USP)
- Written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people
- General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), related pharmaceutical technology, electronic health records (EHR), and EPIC
- Flexibility to orient and work at provider clinics and specialty pharmacies
- Clinical knowledge and therapy management for Chronic Inflammatory disease states/HIV/Oncology
- Maintain excellent written and verbal communication skills and strong critical thinking skills
- Excellent organizational skills, and ability to prioritize/follow-up
- Flexible; able to cope with rapid change; comfortable with an ambiguous environment
- Ability to troubleshoot problems efficiently and effectively
- Capability to set priorities amongst a series of competing clinical and operational priorities and to effectively coordinate and direct the work of others to meet specified deadlines
- Proficient computer experience and accurate data entry skills in MS Word and Excel
- Must be able to lift 50 lbs
Desired Qualifications
- Experience as a specialty pharmacist in a hospital setting
- Board Certification (ex: BCACP/BCPS/BCOP/CSP)
- EPIC and Beacon experience
- ASHP Post-Graduate Year 1 ASHP General Pharmacy Residency
