Job Summary

Serves as a patient advocate in an ambulatory setting, navigating care settings and addressing coordination needs for high, moderate, and low-risk patients. Reviews records and reports to identify patients requiring care coordination, then assesses and documents clinical, psychosocial, or financial barriers. Consults with multidisciplinary teams to develop and evaluate care plans, providing triage support and educational guidance on procedures, medications, and wellness. Ensures timely continuity of care by managing referrals, facilitating transitions, and coordinating community resources to minimize readmissions. Monitors quality and satisfaction metrics while participating in continuous improvement initiatives. Provides after-hours on-call support for urgent care coordination issues. Requires current Illinois RN license, BSN, and three years of care coordination experience with chronic conditions.

Required Qualifications

  • Current license as a Registered Nurse - State of Illinois Professional Nurse
  • Bachelor of Nursing Degree (BSN)
  • three to five years of relevant clinical experience
  • BSN required within two years of hire
  • 3 years of experience working with chronic conditions
  • experience with evidence-based medicine
  • experience with care coordination
  • experience with care management
  • experience with psychosocial and behavioral factors affecting health outcomes
  • Willingness to travel (some travel may be required)
  • Background Check authorization and disclosure form

Desired Qualifications

  • 5 years preferred experience
  • 3 years of experience in Care Management or Care Coordination
  • Case Management certification
  • willingness to complete Case Management certification upon meeting eligibility criteria
  • Experience with ambulatory operation and ambulatory nursing care
  • Experience with process improvement
