Job Summary
Collaborate with PCPs and specialists to develop and implement care plans for heart failure patients, providing direct patient care and oversight to clinical staff. Assess physical, psychosocial, and emotional needs while ensuring clinical protocols and documentation standards are followed. Deliver patient education and support, including IV starts and procedure scheduling, to keep patients healthy and out of the hospital. Monday-Friday schedule with no weekends, flexible hours 7:00 AM-3:30 PM. Requires RN license, 2-5 years experience, and BLS certification. Preferred Bachelor's degree and 5-10 years experience. Required Qualifications
- Nursing Diploma/Associate's degree in Nursing
- 2-5 years of experience
- No Supervisor Experience
- Basic Life Support (BLS) certification
- RN license
- Experience with Cardiology or heart failure patient care
