Ambulatory Medication Access/Referral Coordinator

Job Summary

Review and process patient referrals, manage insurance authorizations, and coordinate scheduling for ambulatory pharmacy services. Maintain accurate EMR records, evaluate patient eligibility for 340B programs and financial assistance, and conduct patient interviews to ensure timely medication access. Collaborate with providers, pharmacies, and payers to resolve access barriers and manage uninsured patient enrollment in charity programs. Review complex cases with pharmacy leadership as needed.

Required Qualifications

  • High school diploma or equivalent
  • Current State of IL pharmacy technician license in good standing
  • Certified Pharmacy Technician (CPhT) credential
  • Minimum 3 years of experience in ambulatory care, specialty pharmacy, or 340B settings
  • Proficiency in electronic medical record (EMR) systems and insurance authorization platforms
  • Knowledge of HIPAA regulations and patient confidentiality standards

Desired Qualifications

  • Working knowledge of pharmaceutical foundation assistance programs and manufacturer co-pay card enrollment processes
  • Experience qualifying and processing patients under 340B drug pricing program guidelines
  • Strong understanding of commercial, Medicare, and Medicaid insurance plans and formulary requirements
  • Experience coordinating patient outreach and scheduling in a high-volume clinical setting
  • Excellent communication skills with the ability to collaborate effectively with providers, pharmacies, payers, patients, and external offices
  • Strong organizational skills with the ability to manage multiple cases and meet deadlines in a fast-paced environment
