PRN Ambulatory Clinic RN Care Coordinator in Cardiothoracic Surgery. Responsible for patient flow, triage and direct patient care; liaison among front desk, nursing staff, physicians, and clinic manager; develop expected patient outcomes, resource utilization, and goals with providers; provide nursing support/coordination to optimize patient outcomes and quality of life; educate patients and families; document assessments and plans in EMR; manage high-risk patients and complex care needs; ensure communication of orders and results to physicians; maintain patient confidentiality and adhere to practice standards. Requires RN licensure with multi-state eligibility, BLS certification, and BSN qualifications as applicable.
