Job Summary
Manage patient flow, triage, and direct patient care while coordinating healthcare services and evaluating patient outcomes. Liaise between front desk staff, nursing personnel, physicians, and the clinic manager to ensure clear communication and support the assigned physicians during clinic sessions. Develop expected patient outcomes, define resource utilization, and coordinate continuity of care following hospital admissions, discharges, and ER visits. Support patient self-management of disease, educate patients and families on medications, nutrition, and diagnostic procedures, and verify all orders are completed. Utilize electronic medical records to document assessments, clinical findings, and progress toward goals while maintaining patient confidentiality.
Required Qualifications
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
- Must have current active multistate state Registered Nurse (RN) license in good standing in state in which you live
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) within 14 days
- High School Graduate
- Bachelor Degree Nursing
- 2 or more years of RN experience
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department
- Basic typing and word processing ability
- Excellent analytical, critical thinking and problem-solving skills
