Job Summary
Ambulatory Clinic RN Care Coordinator role overseeing patient flow, triage, and direct patient care in Orthopedics and Sports Medicine. Responsibilities include liaising between front desk, nursing, physicians, and clinic manager; developing patient outcomes and resource utilization with providers; nursing support and coordination to optimize patient outcomes and quality of life; supporting self-management of disease; coordinating continuity of care post-discharge; managing high-risk patients with multiple comorbidities; educating patients and families about medications, procedures, and safety; documenting assessments, plans, and progress in EMR; ensuring orders and results are communicated to physicians.
