Job Summary

Manage patient flow, triage, and direct patient care while coordinating healthcare services for the Advanced Heart Failure ambulatory clinic. Liaise between front desk staff, nursing personnel, physicians, and clinic managers to relay patient information and ensure understanding of the plan of care. Develop expected patient outcomes, define resource utilization, and support patient self-management of disease through clinical and educational programs. Coordinate continuity of care following hospital admission, discharge, and ER visits, utilizing EMR to document assessments, clinical findings, and treatment progress. Respond to incoming phone calls, educate patients and families on medications and diagnostic procedures, and maintain patient confidentiality.

Required Qualifications

  • Bachelor Degree Nursing
  • 2 or more years of RN experience
  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
  • current active multistate state Registered Nurse (RN) license in good standing in state in which you live
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) within 14 days
  • Basic typing and word processing ability
  • Excellent analytical, critical thinking and problem-solving skills
