Job Summary
Nurse Navigator – Ambulatory Care in the Heart Failure Clinic provides coordination and support for patients with heart failure across the continuum of care.Responsibilities include assessing patient needs, delivering education, coordinating individualized care plans with providers and interdisciplinary teams, monitoring patient progress, facilitating transitions of care, and ensuring adherence to treatment plans. The role supports quality, safety, and operational effectiveness, documents in EPIC, and contributes to program growth and community awareness. Requires BSN, current RN license, and 2–3 years nursing experience; PRN/clinic RN in Mercy Hospital South with location in St. Louis, MO.
Required Qualifications
- Graduate from an approved school of nursing
- Current RN license in the state of practice
- Experience: 2–3 years of nursing experience required
- Bachelor of Science in Nursing (BSN)
