Ambulatory Care Pharmacist

Job Summary
Conduct comprehensive medication therapy management and disease state management for patients with chronic conditions such as diabetes, hypertension, and heart failure in an ambulatory care setting. Assess medication regimens for appropriateness, safety, and adherence while identifying and resolving medication-related problems through direct patient care and telehealth encounters. Collaborate with physicians, nurses, and other providers to establish individualized treatment goals, optimize therapies, and educate patients and staff on self-management strategies. Document clinical assessments and interventions in the electronic health record, support quality improvement initiatives, and mentor pharmacy students and residents.
Required Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited college or university
- Successful completion of an accredited PGY1 Pharmacy Residency program
- Demonstrated clinical experience in either a hospital-based or ambulatory care pharmacy practice setting
- Strong knowledge of pharmacotherapy, chronic disease state management, and evidence-based clinical practice guidelines
- Ability to assess, monitor, and optimize medication therapy to improve patient outcomes and medication safety
- Excellent interpersonal, communication, and patient counseling skills, with the ability to effectively communicate complex clinical information to patients, caregivers, physicians, and other healthcare professionals
- Proficiency with electronic medical records (EMR) and clinical documentation systems
- Strong critical thinking, problem-solving, and organizational skills with the ability to work effectively within a multidisciplinary healthcare team
