Job Summary
Ambulatory Care Manager Pediatrics oversees care management and care coordination for pediatric and adult patients with complex illness in a primary care setting. Responsibilities include conducting patient assessments, developing patient-centered care plans, coordinating care across settings, monitoring outcomes, and guiding high-risk patients to self-management to prevent hospital utilization. The role requires collaboration with ambulatory care management teams, primary care leadership, specialists, and families, applying evidence-based guidelines to optimize health status. The position manages a caseload of approximately 150 complex patients, emphasizing prevention, discharge planning, and longitudinal care, with a focus on quality, efficiency, and payor/clinician collaboration. The ideal candidate holds a BSN/BS, is a licensed RN in Michigan, and demonstrates strong leadership, communication, and analytical skills, with proficiency in standard software and the ability to work autonomously.
Required Qualifications
- Bachelor's degree (BSN or BS) or enrolled in BSN program with completion within 6 months of hire
- Two to five years of nursing experience with pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting
- Licensed RN in good standing with the State of Michigan
- Must maintain current CPR certification and follow department standards and Bronson policy for maintaining and staying current with BLS through RQI simulation
- Ability to utilize word processing, spreadsheet, presentation programs, databases, and other software relevant to the job
- Demonstrates customer focused interpersonal skills to interact with practitioners, interdisciplinary health care team, community agencies, patients, and families
- Demonstrates ability to work autonomously and be directly accountable for practice
- Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization
