Job Summary
Ambulatory Care Clinical Pharmacist responsible for providing Comprehensive Medication Management (CMM) in collaboration with hub-based and virtual care teams. Develop medication management action plans, ensure safe and cost-effective drug therapy, conduct ongoing assessments and refinements of care plans, perform medication reconciliation, educate clinical and non-clinical staff, and help transition to collaborative practice agreements (CPA) with Cityblock physicians to enable pharmacist-led member assessments and potential ordering of labs and medication adjustments. Collaborates with internal and external providers to optimize care for chronic diseases and preventive care, with a focus on improving population health outcomes and reducing avoidable ED visits and hospitalizations.
Required Qualifications
- Active Pharmacist license in MI, IL, or OH
- Willingness to pursue cross-licensure as needed
- PGY1 residency and PGY-2 Ambulatory Care Residency, or 2+ years ambulatory care experience
- Ability to obtain Ohio licensure upon hire (company-supported)
- Ability to provide Comprehensive Medication Management (CMM) in collaboration with care teams
