Ambulatory Care Clinical Pharmacist - Ambulatory Care Clinical Pharmacist – Remote (Ohio License Required)

Provide Comprehensive Medication Management (CMM) in collaboration with hub-based and virtual clinical care teams, co-designing individualized therapeutic regimens for chronic disease states and preventative care. Contribute to medication management action plans with measurable parameters and timelines, while communicating with external providers to ensure clinical interventions align with member goals. Support the clinical team with drug information, dosing, and interaction avoidance, and participate in staff education regarding the Commons platform and post-discharge reconciliation. Assist in developing disease state-specific clinical pathways to transition toward collaborative practice agreements. Requires a PGY1 residency, PGY-2 Ambulatory Care residency, or two years of ambulatory care clinical experience. Salary range $140,000 - $150,000.
