Job Summary
Conduct restorative, outreach-based guidance in the client's home environment while managing a shared caseload for adults with complex psychiatric issues. Collaborate with family, network partners, and a multidisciplinary team to co-create and execute treatment and guidance plans. Flexibly respond to crisis situations and support clients in regaining control over their daily lives across housing, finances, and social participation. Work within a flexible assertive community treatment model that prioritizes proximity to the client's lived world.
Required Qualifications
- een afgestudeerde HBO GGZ-Agoog bent
- Een Verklaring Omtrent het Gedrag (VOG)
- affiniteit hebt met de doelgroep van Ernstig Psychiatrische Aandoeningen
