Coordinate treatment plans and conduct intakes for children and young adults aged 14 to 24 with complex psychiatric issues, serving as the primary case manager and first point of contact. Deliver psychiatric, somatic, and medication-based care in a restorative, outreach-focused model, working intensively with the individual, their network, and multidisciplinary subteams of three to four colleagues. Manage your own caseload by scheduling appointments and meetings independently, while identifying needs to involve other professionals and prevent hospital admissions during crises. Perform home visits and office-based care to support recovery across multiple life domains.
