Conduct restorative care for adults with severe psychiatric disorders in their own living environments, visiting clients at home, on the streets, or in preferred locations. Collaborate within a multidisciplinary team to establish treatment plans, evaluate progress, and support daily structure, social participation, and self-determination. Identify crisis situations and coordinate with family members, caregivers, and other healthcare providers to ensure continuity of care. Manage a shared caseload while maintaining flexibility between independent work and team collaboration.
