Job Summary
Conduct intensive, community-based rehabilitation care for adults with serious psychiatric disorders through home visits and on-site appointments. Manage caseloads by initiating and maintaining networks with local partners including protected housing, social organizations, and police. Facilitate crisis interventions, intake conversations, and systemic treatments while collaborating closely with multidisciplinary teams and family members. Operate within a flexible schedule at the Van Hilligaertstraat location, requiring BIG-registered nursing qualifications and at least two years of outpatient mental health experience.
Required Qualifications
- BIG-registratie als HBO verpleegkundige
- afgeronde opleiding HBO maatschappelijk werk & dienstverlening
- HBO sociaal pedagogische hulpverlening
- HBO social work (uitstroomprofiel Zorg)
- afgeronde post-HBO opleiding tot GGZ-agoog
- BIG geregistreerd beroep
- minimaal 2 jaar ambulante ervaring in de GGZ
- werken op onze locatie in De Pijp, aan de Van Hilligaertstraat
- werk tijdens kantooruren
