Job Summary
Conduct outpatient nursing or treatment support for clients with complex psychiatric conditions in the FACT team in Oss. Go to clients in their living environment to coordinate care, signal changes in mental health, and lead partial treatments while maintaining a shared caseload with the IHT team. Coordinate with treatment providers, family members, and external partners, acting as a primary contact point for clients. Perform planned weekend visits once every six weeks in collaboration with the IHT team. Work within a multidisciplinary team of approximately 15 colleagues using a shared caseload and recovery-oriented approach.
Required Qualifications
- Een hbo-diploma Verpleegkunde met een geldige BIG-registratie
- een hbo-diploma MWD, SPH, Social Work* of vergelijkbaar
- Ervaring in een zelfstandige, ambulante functie
- Ervaring met of affiniteit met de EPA-doelgroep
- Je bent beschikbaar voor weekenddiensten (1x per 6 weken)
- Je hebt een rijbewijs en eigen vervoer
