AlmaVia of Camarillo - Health Care Coordinator

Direct resident care and administration of medications per the care plan, training and oversight of Medication Assistants/Technicians, ensuring medication distribution accuracy per physician orders and regulatory documentation; assist with admissions and discharges, perform admissions valuation, ensure medical records and medical orders are current; conduct assessments when conditions change and coordinate with the medical team for intervention and family notification; collaborate with Resident Services Director and Resident Assistants to meet resident needs; respond professionally to resident inquiries; monitor medication distribution and perform daily rounds to align care with the service plan, including resident grooming and nutrition documentation; perform licensed treatments as required; adhere to policy/procedure and participate in ongoing duties as assigned.
