AL Care Provider

AL Care Provider responsible for direct neighbor care including activities of daily living (bathing, grooming, toileting, laundry, meal service) and support services per care plan. Responsibilities include performing documentation per regulations, coordinating medical/non-medical appointments and transportation, providing privacy and dignity, and communicating with physicians, families, and case managers. The role requires assessment with glucometer and blood pressure equipment, maintaining HIPAA-compliant confidentiality, and collaborating with team to ensure compassionate, patient-centered care. Shifts are 8-12 hours, in-person at Sterling, CO location, with potential holidays/weekends and overtime as needed. Healthcare environment with equipment such as glucometer, blood pressure devices, weight scales, lifts, wheelchairs, oxygen tanks; strong written and interpersonal skills are essential.
