Job Summary
Coordinate and complete assessments for adults with intellectual disabilities, assess member progress against set objectives, and develop individual goals aligned with their Individual Service Plans. Participate in ISP meetings, act as the Plan lead for development and annual updates, and report discrepancies or changes in member needs. Ensure all services and documentation comply with state regulations and company policies by reviewing monthly participation records and preparing case notes. Support Center staff and members with direct service needs, crisis intervention, and training on health and safety protocols. Maintain required licenses and demonstrate proficiency in client techniques to uphold Active Day's culture and philosophy.
Required Qualifications
- An associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities
- A bachelor's degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities
- A master's degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities
- Minimum one (1) year experience working with adults with intellectual disabilities preferred
- Must be extremely detail-oriented with exceptional organizational skills in order to maintain compliance
- Strong communication and relationship building skills
- Ability to multi-task and manage competing priorities
- Working knowledge of Microsoft Office (Outlook, Word and Excel)
- Successful results of background check (and successful MVR results for Hamilton OH centers only) and any other state-specific requirements
