Job Summary
Provide direct personal care to terminally ill patients, including assistance with hygiene, grooming, toileting, safe mobility, and meal preparation. Observe and report changes in patient condition while offering emotional support to patients and families. Maintain accurate documentation of visits and attend team meetings for Quality Improvement activities. Practice infection control and follow safety protocols at all times. This safety-sensitive position requires an active CNA license, CPR certification, and a driver's license. Work independently in home settings, assisted living facilities, or inpatient units. Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position
- Employee must meet minimum requirements to be eligible for benefits

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
- Exposure to end-of-life care, wound care, or admissions support
