Job Summary
Provide direct personal care to terminally ill patients, including assistance with hygiene, grooming, toileting, safe mobility, and meal preparation. Observe and report changes in patient condition while offering emotional support to patients and families. Maintain accurate documentation of visits, attend team meetings, and practice strict infection control protocols. This full-time dayshift role requires an active CNA license, current CPR certification, and a driver's license with reliable vehicle insurance. Six months of direct patient care experience is required. Heartland Hospice, part of Gentiva, delivers compassionate care across nearly 600 locations in 38 states, supporting patients and families through end-of-life journeys in homes, assisted living, or inpatient units.
Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
- Exposure to end-of-life care, wound care, or admissions support
