Job Summary
Provide direct personal care to terminally ill patients, assisting with hygiene, grooming, toileting, safe mobility, meal preparation, and light housekeeping. Observe and report changes in physical or mental condition while offering emotional support to patients and families. Maintain accurate visit documentation, attend team meetings, and practice strict infection control protocols. Requires active CNA license, CPR certification, driver's license, and six months of direct patient care experience. Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
