Job Summary
Provide direct personal care to terminally ill patients, including assistance with personal hygiene, grooming, toileting, safe mobility, meal preparation, and light housekeeping. Observe and report changes in patient conditions while offering emotional and psychological support to patients and families. Maintain accurate documentation of visits, attend team meetings, and practice infection control and safety protocols. Maintain an active CNA license, current CPR certification, and a driver's license with reliable vehicle insurance. Six months of direct patient care experience is required.
Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position
- Employee must meet minimum requirements to be eligible for benefits
- Where applicable, employee must meet state specific requirements

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
- Exposure to end-of-life care, wound care, or admissions support
