Job Summary
Provide direct personal care to terminally ill patients, including assistance with hygiene, grooming, toileting, safe mobility, and meal preparation. Support patients and families with emotional and psychological care while maintaining accurate visit documentation and observing physical or mental condition changes. Practice strict infection control and safety protocols during all visits. Attend team meetings and participate in Quality Improvement activities. This role requires an active CNA license, current CPR certification, and a driver's license with reliable vehicle insurance. Six months of direct patient care experience is required. Work independently in home settings, assisted living facilities, or inpatient units.
Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position
- Employee must meet minimum requirements to be eligible for benefits

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
- Exposure to end-of-life care, wound care, or admissions support
